The Flinders Chronic Condition Management Program (Flinders Program) has been developed as a structured process and includes a set of tools for clinicians to use to support client self-management. The Flinders Program continues to evolve with recent adaptations developed for use with targeted populations such as indigenous people or with veterans.

The upskilling session provides the opportunity for those who have been previously licensed in the Flinders CCM Program to update their knowledge of current Flinders Program content and adaptations including:
